POG Health Card and POG AI Services

Prognostic Optimization Group, Inc. (“POG AI,” “we,” “us,” or “our”) provides the POG Health Card mobile application, the POG AI web application, and related websites and services (collectively, the “Service”). The Service allows you to create a personal health profile and share it with healthcare providers and others of your choosing by presenting a scannable QR code.
This Privacy Policy explains what information we collect, how we use it, when we share it, and the choices you have. By using the Service, you agree to the collection and use of information as described in this Policy. If you do not agree with this Policy, please do not use the Service.
1. Information You Provide to Us
Account Information. When you create an account, we collect information such as your name, email address, and login credentials.
Health Profile Information. The core purpose of the Service is to store the health information you choose to enter so that you can share it. Depending on what you choose to include, this may consist of your date of birth, sex or gender, medications and dosages, allergies and reactions, medical conditions and current treatments, surgical history, family medical history, social and lifestyle information, immunization records, care-planning documents you choose to upload (such as a healthcare proxy or advance directive), and any other information you choose to include in your profile.
You decide what information to include in your profile. You can add, edit, or remove information at any time through the app or the web application.
Communications. If you contact us for support or send us feedback, we collect the information contained in those communications.
2. Information Collected Automatically
Like most online services, our servers automatically record certain technical information when you use the Service, such as your IP address, device and browser type, operating system, and the dates and times of your activity. We use this information to operate, secure, troubleshoot, and maintain the Service. The web application may use cookies or similar technologies that are necessary for the Service to function, such as keeping you signed in.
Our public website also uses Google Analytics to help us understand overall visitor traffic, such as which pages are viewed and how visitors reach the site. This information is used in aggregate form and is not linked to your health profile.
3. How We Use Your Information
We use the information we collect to:
- Provide, operate, and maintain the Service, including generating your health summary and QR code;
- Create your account and authenticate you when you sign in;
- Display your health profile to the people you choose to share it with (see Section 4);
- Respond to your questions, feedback, and support requests;
- Protect the security and integrity of the Service and prevent fraud and abuse;
- Send you important notices about the Service, such as changes to features, security, or this Policy; and
- Comply with applicable legal obligations.
We do not sell your personal information. We do not use your information for third-party advertising, we do not share your information with data brokers, and we do not use your information to track you across other companies’ apps or websites.
4. How Your Information Is Shared
Sharing You Direct (QR Code Sharing). The central feature of the Service is sharing your health profile by allowing another person — such as a healthcare provider — to scan your QR code. When your QR code is scanned, the person or organization scanning it can view the health profile information associated with that code. No account or login is required to view a profile shared this way. Only share your QR code with people and organizations you trust. Once someone views your information, POG AI cannot control how they use, record, or retain it.
Service Providers. We use trusted third-party vendors to help us operate the Service, such as cloud hosting and infrastructure providers. These vendors may process your information only as necessary to provide services to us and are required to protect it.
Legal Requirements. We may disclose information if we believe in good faith that disclosure is required by law, regulation, legal process, or governmental request, or to protect the rights, property, or safety of our users, the public, or POG AI.
Business Transfers. If POG AI is involved in a merger, acquisition, financing, or sale of all or part of its assets, your information may be transferred as part of that transaction. We will notify you of any change in ownership that affects your personal information.
5. Data Retention and Deletion
We retain your information for as long as your account is active or as needed to provide the Service. You may delete individual items of information from your profile at any time. You may also delete your account entirely through the Service or by contacting us at the address in Section 12. When you delete your account, we will delete your personal information within a reasonable period, except where we are required to retain it to comply with legal obligations, resolve disputes, or enforce our agreements, or where it resides in routine backups that are purged on a regular schedule.
6. Your Choices and Rights
You may access and update the information in your profile at any time through the app or the web application, and you may delete individual entries or your entire account as described in Section 5.
Depending on where you live, you may have additional rights under applicable privacy laws, such as the right to know what personal information we have collected about you, to request a copy of it, to request that we correct or delete it, and to not be discriminated against for exercising these rights. To exercise any of these rights, contact us using the information in Section 12. We will verify your request and respond as required by applicable law.
7. Security
We use reasonable administrative, technical, and physical safeguards designed to protect your information, including encryption of data in transit. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security. You are responsible for keeping your login credentials confidential and for choosing the people and organizations with whom you share your QR code.
8. Children’s Privacy
The Service is intended for use by adults. A parent or legal guardian may create and manage a health profile containing information about their minor child — for example, for use at school, camp, or medical visits — and is responsible for the accuracy of that information and for deciding when to share it. We do not knowingly collect personal information directly from children under 13 without verifiable parental consent. If you believe a child under 13 has provided personal information directly to us, please contact us using the information in Section 12 and we will delete it.
9. Important Notes About Your Health Information
The information in your health profile is provided by you (or by the account holder managing the profile). The Service is offered directly to consumers and stores information you provide; it is not a medical record system maintained on behalf of your healthcare provider or health plan. Your profile is intended to help you share information with providers you choose, and it is not a substitute for the official medical records maintained by your providers or for professional medical advice, diagnosis, or treatment.
10. International Users
The Service is operated from the United States. If you use the Service from outside the United States, you understand that your information will be transferred to, processed, and stored in the United States, where privacy laws may differ from those in your jurisdiction.
11. Changes to This Policy
We may update this Policy from time to time. If we make material changes, we will post the updated Policy with a new effective date and, where appropriate, provide additional notice through the Service. Your continued use of the Service after changes become effective constitutes acceptance of the updated Policy.
